road traffic accidents not a menace but terrifying [if you understand the magnitude), the PIL in supreme court of India

"Over the last decade, a few thousand Indians have lost their lives as a consequence of terror attacks. An estimated 15,000 people die every year due to malaria. But 1.2 lakh Indians lose their lives each year on the roads. And 10 times that number is seriously injured."

this is the quote from the Hindustan Times under the article "this neglect is tragic"

helmet for the pillion rider

most of us agree that helmet use is good for us, if in doubt refer to this link which is evidence based

however We often see drivers of motorcycles  riding without helmet in a country like India

hence you might ask why bother with the pillion rider!

however this blog is for the sensible motorcyclists who do wear helmets

we do see quite a number of motorcyclists wearing helmet but the pillion riders do not wear

often the pillion riders happen to be the spouses or children of the motorcyclists or friend or a relative


the safety is the same whether you are driving or pillion riding a motorcycle

saety tip for pedestrians on the road. walk on the right hand side of the road where there are no foot paths

In India in particular, we see lots of pedestrians using main roads.

as a car driver, one tends to think that the roads are designed for four wheelers.

in other words, two wheelers are taken as a nuisance

then imagine the case of pedestrians!

a car driver or bus or lorry driver often is blinded to these important road users.

then what precautions does one, as a pedestrian need to take to avoid being hit by these two or four or more wheelers

the paradox of lighting, safety in driving and human eye

Most people use brighter lighting for better vision and use high beam light whilst driving

makes sense but

more the bright the lighting the eye adapts

it lets in only minimum of bright light by constricting the pupil

the pupil is the hole in the middle of a filter of light in our eye



letting in minimum of the bright light increases the comfort

but what it also does reduces the field of vision

field of vision is the area in front, your eye can see 


coming to the point, whilst driving, when people use high beam,  it reduces the vision of the driver opposite to you

if the driver opposite to you cannot see you well, then it is not good news for you


makes sense?


hence do use the high beam lighting whilst driving only when there are no other vehicles in front of you

dip the light when there is an oncoming vehicle

safe driving tip

There is a statement written at the back of all the buses in Tamil Nadu

requesting the drivers behind to leave a gap of 10 metres

meaning that this would be a safe distance in case of sudden braking by the bus ahead

that you have ample time to stop your vehicle before it hits the bus in front



makes sense eh!

but how would one calculate the distance while on the move to be 10 metres

easier way is the two second rule

gram negative organisms, they are bacteria present commonly in faeces, concept of BCP

Not a pretty topic for a blog eh!

but important to know for our day to day life

The fact as it was:    when we did our medical school, we were taught that the infections causing lung infection called pneumonia is caused by a bacteria called pneumococcus.

The fact as it is:  but now a days, we are facing infections of lung caused by so called gram negative organisms!

the question:  but how does such an organism/bacteria which is usually present in faeces find its way to our lungs?

the answer:  bacteria reach our lungs obviously by air

                     they need little aeroplanes to travel in the air

                     These are called Bacteria carrying particles

The dust in the air offers nice vehicles for their travel

But if the faeces are not disposed appropriately, then there is source and the dust in the air act as little aeroplanes for them to reach us.

Apparently people with wisdom do not get angry!!!!

Yet Subramania Bharathi asks us to learn to get angry by saying Roudram Pazhagu

I was taught by one of my senior colleagues that this means that you learn to show anger but do not get angry. 

This makes sense as when one gets angry the health gets affected and one does look like a fool!

But to show anger gets the point across!

another one bites the dust! Road traffic accidents with SUV roll over with occupants, no seat belt, left with paralysis of legs

the first part of the title is from Queen and it is one of the all time great songs.

not nice to see the outcome of another accident with this title of the Queen Song.

not wearing seat belt is an offence in some countries.

seeing a patient who cannot move both legs, called paraplegia is not nice.

as the patient with inability to move both legs, will not recover the ability to walk most of the tims

and to know that this would have been prevented if one was wearing seat belt is even harder to swallow.

then to realize most of the people who travel in big vehicles such as bus and SUVs do not wear seat belts.

probably they feel happier not wearing seat belts, as the vehicle is big and they feel safe inside.

probably some of the vehicles such as bus do not have provision for seat belts


how can as a doctor explain that this irreverisble loss of use of both legs could have been prevented if they were wearing seat belt.


hence i am writing this blog, so that one is not in this situation ever in the first place

What to expect after total knee replacement? myths about knee pain and its treatment options.

Knee pain is not due to ageing alone, and knee pain is not incurable either!

19 year old Swetha (name changed) a college student is not able to walk to her college due to knee pain. She is not able to start the day comfortably due to severe stiffness of her joints. Her sleep is often disturbed. She has severe knee arthritis due to condition called Juvenile Rheumatoid arthritis.


68 year old Kamatchi (name changed), has intermittent knee pain which suddenly catches unaware. She feels as though her knee might buckle and she might fall. In between episodes, she has some ache, but mostly pain free. She suffers from a Cartilage tear with early arthritis.


Pain is present in most of the adults and joint pain is the commonest cause of pain in humans. Knee in particular causes so much pain and disability in adults especially ladies in India. Most people think that this is due to ageing. However the two real life stories described above tells you that how this is not entirely true. What is more important is to determine the cause of the pain to decide the best course of action.

To know the cause of the pain, it helps us to learn a bit about this joint which is essential for our day to day life. Knee joint is a hinge joint like a door which moves in one plane of bending. This is made of three bones covered with cartilage which provides a smooth gliding surface. There are thick cartilage called menisci which shares the load transfer. Ligaments balance the bones in movment such as sitting and walking. Muscles move the joint to facilitate the movement. There are fluid filled sacs in and around the knee to lubricate just like the engine oil.

When knee hurts due to any of the above tissues, it restricts the function of the knee. People are not able to walk freely or climb stairs nor sit in the toilets or the floor cross legged. Pain disturbs the sleep and rest may not relieve the pain. Patients have accepted to crying due to the pain and are confined to their houses with inability to attend to their social activities.

Pain due to specific arthritis such as Gout and Rheumatoid arthritis can be treated well with medication when detected early. Mechanical problems affecting the soft tissues of the knee can be resolved with simple life style modifications, physiotherapy, weight reduction, diet modifications etc. Your doctor would be able to diagnose these problems with talking to you and examining alone.
Sometimes, investigations such as X ray and MRI scan can help us to pin point the cause of the problem. Severe arthritis will be usually detected on x ray itself though MRI scan may be necessary in some patients.

Severe Arthritis affecting one's life can be very successfully treated with treatment of joint replacements. Patients can return to their normal day to day life and social commitments. Not everyone will need joint replacement and some of the patients are successfully treated with key hole surgery called arthroscopic surgery.

the 19 year old Swetha is  now a happy person, cycling to her college after successful knee replacement. in contrast the 68 year old lady's life has been dramatically changed by key hole surgery. A 50 year old lady who works in Governement noon meal scheme is happily cooking for the children with artificial knee. She is so happy with this artificial knee that she chose to come have her other knee replaced when it started giving her trouble. 56 year old
Sub inspector is happily doing his job and on the move thanks to the technology of artificial knee replacements.

The scenarios described above are not stories but real life history of patients with artificial knee replacements right in our country with the surgery performed to international standards with the best of care one can get.

Water bodies of our country, please fight to preserve them.  

Only nature can fight nature, humans cannot, however  we can disturb nature to our disadvantage

We cannot destroy nature as people tell us;  only the balance can be destroyed.  Only when the nature is in harmony with each other, man can exist safely and comfortably.

Scorching sun, only water can help us to avenge our thirst.

Shade from the stone or earth is efficient in shielding us from the sun.

A nice breeze of wind can take the effect of the sun and can move the clouds away from drenching us with rain. Equally the winds bring us the clouds and in turn the much needed rain.

Heavy rains can neutralize the fire and fire of the sun brings us the light and keeps us warm. We are warm blooded animals and cannot survive without the heat.

 If we harness these natural forces of the planet, we do not have to struggle hard to survive.

That is why the wind energy and solar energy are here to stay; we only need to perfect the way to get it.

Man makes his life better only in the short term by destroying the nature.

 I am referring to some areas in the world, where the natural bodies of water are removed to make way for human inhabitance.

Hence kindly chronicle the water bodies in your area and hoping not to destroy them but live with them. please take photos of how they are at present.

Google maps should have timeline of their maps so that people can refer to how the terrain changes with human interference.

The existing lakes, ponds and rivers are fast disappearing due to human interference or lack of maintenance. . Hope this makes sense

One way is to hand over the existing ponds maintenance to private companies as lease so that they can be well cared for.

Bone and Joint Decade 2011 to 2020: Arthrits awareness article and Total Knee Replacement Surgery


Introduction:

Every other person over the age of twenty suffers from some pain in the body.  Those with pain are most commonly affected with joint pain.  Joint pain due to arthritis restricts one’s activity and as a result causes disability. Knee joint is the commonest major joint affected with arthritis and is widely prevalent among the general population with one in five adults affected.
World Health Organization (WHO) listed Arthritis as one of the four major orthopedic problems affecting millions of people around the world. Bone and Joint Decade 2000 to 2010 was followed by WHO to spread the awareness of this disabling problem. The government of India through its participation in this Bone and Joint Decade has shown its commitment in prevention and treatment of these disabling conditions.
Fortunately advanced medical technology helps us to cure this disabling condition. The purpose of this article is to present the facts and myths of arthritis and total knee replacements.
Normal Knee Joint:
When the knee joint works well, you do not feel the importance of this joint. It gives us stability and mobility when performing normal activities However when it hurts and limits our activity; we understand the role of this joint in our normal day to day life.

Why does our knee gives raise to pain causing inability to do what are essential activities such as walking. We shall dwell into how a normal knee works to understand about arthritis and treatment method of knee replacement.


Knee joint is one of the strongest joints in the body which is vital to our normal mobility. It helps us to walk normally by bending the knee to an angle of sixty degrees. It helps us to climb stairs by bending to right angle. We can squat on the floor or sit cross legged by bending more of the joint.

The knee joint is made of two main bones called Tibia   (the shin bone of leg) and Femur ( bone of thigh). It also has two small bones called fibula and patella (knee cap). These bones are covered with a thick coating called Cartilage. This cartilage is very smooth and allows gliding without friction.

These bones are held together with tissue called capsule and ligaments, yet allowing movement in certain directions and not others (like the hinges of the door). It also has cartilage called menisci which helps the bones transfer the load without undue stress at any one point. The lining of the joint called Synovial membrane secretes fluid to lubricate the joint.

What is Arthritis:
Arthritis is the process where the normal knee starts swelling and painful. This can happen either as one gets old or due to inflammation of the lining of the joint. Arthritis does not always affect elderly. Some people are as young as twenty or thirty needing surgery to cure this disabling condition.
The thick coating of the ends of the bone called cartilage begins to lose its effectiveness. The cartilage starts to crack, delaminate and disappear completely exposing the bare bone. The bare bone when rubbing against the other bone in the joint, gives raise to the pain.
The body works harder by secreting more fluid as a futile attempt to lubricate the damaged joint. Thus the swelling of the joint is more a result of arthritis rather than the cause of pain.
How does arthritis affect us.

Arthritis gives rise to unpleasant sensation of pain. This restricts the activities of normal day to day life of walking, sleeping and getting up from a seated position and climbing stairs. The restriction from going out of the house, using public transport, partaking in family and social engagements is often significant than what is apparent. This disability is well known to affect the quality of our life. The deterioration in our quality of life is measurable to help us to offer a cure for this pain.

The long term success of the knee replacement surgery is more important that looking at short term benefits of this surgery. Any material placed between the end of the femur and tibia will relieve the arthritic pain. What is more important however is the durability of the material in staying without damaging the underlying bone!

The damage to the underlying bone is the worst scenario when faced with the failed knee replacement surgery. One fears this as the damage often starts without any warning such as pain. These techniques are available and used routinely in India and locally in Trichy
Other Joint Replacements

The total joint replacements are commonly done for other joints such as Hip joint and shoulder joint with excellent outcome. Less commonly replaced joints are those of elbow, wrist and ankle, though there are new joints available for practically all our joints.

Summary;

Arthritis is a very common disabling condition affecting majority of general population. There is continuing lack of awareness of arthritis and the curative treatment options such as Joint Replacement surgery. Hence the World Health Organization has extended its mandate for the Bone and Joint Decade for another ten years from 2011 to 2020. Crumple Zone, Bull bars and SUVs: point to ponder

I see more accident victims as an ortho surgeon in India than i did back in UK.


I see more SUVs in India than in UK


In fact for the last one year i am longing to buy a perfect SUV to replace my old car


But i noted more accidents where SUVs are involved than cars.


This made me to ask, are the Big SUVs (which look big, masculine and safer when sits inside or to look from outside) desirable from safety point of view?


I understand Crumple zones in the safety of cars and vehicles. I also noted that most SUVs are fitted with bullbars which make it commanding to look at and safer to think!


 are they really safer.


More i did research and i must admit internet is amazing place and you ask the question, you tend to get answers,

but one must be very careful to interpret the answers.

Even so,

             most answers i get is that combining SUVs with high centre of gravity,
                                                                    Bullbars,
                                                                    Carelessness when driving one ( i understand that psychologically when you drive an SUV, you tend to take less care as you feel safer),
                                                                    Speed with unexpected obstacle ( which you get many in local  Road driving conditions)

       may be a bad combination.

This combination may not be uncommon as one feels invincible driving a big SUV

SUVs and MUVs and the sense of (or lack of) security and safety. Read on!!!!!!!

I was called to attend to four patients following road traffic accident. They all had injuries which were not serious and as i was talking to them, asked them what happened.

One of the patients said, that they were travelling in a sports utility vehicle or multipurpose vehicle. i will not name the make of this jeep styled vehicle for purposes of not eliciting controversy.

Then what happened. a small compact hatchback the make of it will also remain nameless was coming in the opposite side. they had a head on collision.

Then the SUV in which these people were traveling rolled over and fortunately these people survived.


I asked what happened to the hatchback which drove straight on to them. Apparently this hatchback had some damage to the bonnet but stood still. while the SUV rolled over!!!!!!!!!

I was surprised.Naturally one would expect the smaller vehicle to have the greater damage. now go and read article in Wikipedia on

What this link says is that the SUVs have high centre of gravity. They tend to roll over even though the Electronic stability built in. if they face an obstacle, for example. even if the obstacle is small and especially if the obstacle is small. this is called tripping over!!!!!!!!

So be ware of this if you own or travel in a SUV

Apathy, sympathy and empathy: Knowing the difference

Last week in the newspaper i read that few people looted the house of a lady. the lady had let them in to their house showing sympathy as they were thirsty. they had asked for some water or refreshments.

Difficult to swallow the fact that the persons to whom the lady showed sympathy were apathetic when they decided to steal from the same person.

What you show to the other person may depend on the situation, you and the other person.

Let us try not to show sympathy where empathy is needed.

for example when you care for someone being a nurse or a relative

[Apathy may be too difficult to master and may be that is what is needed in some situations and not sympathy. unfortunately more people are showing apathy where they should be sympathetic or empathetic]

what type of exercise do i need: a rough guide

Start with flexibility exercise: good for all the joints and the muscles. this takes away any unnecessary aches and pains

to start, follow the link on flexibility on the links section of the blog for this

then with core stability.: good for spine

both these are addressed with Program such as yoga

then go for aerobic exercises; good for heart, lungs and general well being. this can be walking or cycling or jogging or aerobic program in a gym

then go for strengthening exercises: these make you look good and helps you to be able to deal with work which needs strength

then do resistance exercises and eccentric training: good when you want the muscles to work for a longer period and this helps in training them.this is helpful for your endurance when you do sports

disposable income vs discretionary income and the alarming trend!!!


Disposable income is income you have left behind after you pay your taxes.

Discretionary income = Gross income - taxes - all compelled payments (bills)

This discretionary income is the income you use for saving for the future and spending today.

Now look at your electricity bill among the compelled payments for example
I paid 1768 for 760 units in 2011 and 3335 for the same amount of units in 2012. Now my discretionary income has reduced dramatically. This is alarming, is it not.

The money I have to save or to spend has reduced even though I have not consumed any more electricity.

Now add the increase in your spending on other utilities and essentials. Groceries, gas, petrol and diesel etc

Worrying is it not!

quality of life vs standard of life and knowing the difference

I want an ipad since it would be nice to have one, eh!!

I have a mac and iphone 3 both more than six years and i want to have new one of both and i can buy them.

I want to watch the latest movie in my LED when the dvd version comes out and i can buy both led tv and a blue ray player

I do not want to wash my dishes and i would buy a dishwasher

Can i have the house of my dreams built and yes i can and i can do all the above, no problems,  money does not seem to be an issue!!!

This is the standard of life i desire and i work hard to reach there and earn my dreams!!!

But this is not the same as quality of life!!!!!!!!!!!!!!!

Can i walk up stairs without pain and feeling breathless

can i walk along the beautiful beach for half an hour and enjoy the nature

can i wash my own back by reaching without having pain

can i go and play a game of tennis

can i bend down to pick up my child without my back hurting

this is what we are talking about, the quality of life

next time we meet, talk about our quality of life and not the standard of life.

lack of flexibility of joints, the commonest reason for pain

I see quite a lot of patients who have pain in the arms, legs, back and neck. most of the pain is due to lack of flexibility of joints due to our current style.

Sitting on the floor , squatting, bending etc are activities one did as a child and yet we have forgotten how to do now.

most pain can be relieved by keeping all the joints of our body move through full range.

how to reach "feeling of enough" when we eat. tips on how not get overweight and obese

Reaching a feeling of satiety is when we stop eating more. obviously if we eat more, stomach gets filled and we say enough. however the stomach is a big bag. it can hold quite a lot of food before we reach the sense of enough or satiety.

hence the only way we can control our weight and keep ourselves fit is to fill our stomach completely with the right kind of food.

one example is to reach 400 calories with oil, it will fill only the bottom pit of stomach. to reach the same amount of 400 calories with chicken , we need to fill quarter of the stomach. with these methods, the stomach does not get filled and we would like to eat more.

however if we fill the stomach completely with fruits and vegetables, entire stomach gets filled and we feel satisfied. yet the calories we have is only 400 calories.

hence please do not reduce the amount of food we eat, but eat plenty of right food.

the reason is that we can only say enough if the top part of stomach gets filled with food. hope this helps

Heel pain prevention and Orthopaedic treatment.

Heel pain is common in India. It is as common as having back, knee and shoulder pain.

Often this is attributed to lack of flexibility in the foot, aggravated by walking on very hard floor.

Walking outdoors or in surfaces with variation in the terrain helps us to put all the joints of the foot to use.

Hard floor which may look nice and easier to maintain, limits the weight transmission to some aspects of the foot.

Heel then takes the brunt of our weight on to the hard floor.

What to do to avoid heel pain?

Wear any cheppal indoors when walking on hard floor

Keep doing some flexibility exercises.

More important spend some time walking on uneven surface on bare foot, may be go out for a walk in a pair of good trainer shoes.

Sometimes this pain can be refractory and you need to see an Ortho Surgeon.

We now have facilities to treat this pain with the proven treatment methods using injection or endoscopic methods.

advantage of surgery to fix the fractured bone

The concrete of our body which when heals from a fracture takes time.

During this time, the patient will have pain and loss of use of the part of the body.

This is acceptable to rest the building which is being repaired for few weeks before you start using it

But for us, pain remains partly even with plaster and rest.

Loss of function of the part of the body for few weeks was acceptable in those days, but people want to return to normality as quickly as possible

The drawback of not being able to use the leg or the arm is immense, understood by the person who has fractured it

Thus surgery helps to fix the bone and keep it still. This relieves the pain and helps to start using the arm or the leg and so on.

the Reinforced concrete of our body, the bone has fractured, what to do

When the Reinforced Concrete of a building breaks, you want to repair it. until then you protect the building by not putting any force.

This is exactly what you do when the bone fractures. You stop putting any force acting across it. meaning you rest the part which is fractured.

Put some ice fomentation, and keep the body part still with bandage or splint with piece of wood.

Repair, the bone does it itself.

But while it repairs, it puts the concrete in the fluid form which then sets like the concrete mixture of water, cement and the sand.

This process in building can take days to weeks and you wait by not putting the stress.

In the bone also, you have to rest the part by immobilizing in a plaster cast or by surgery.

The body then puts blood at the fractured site which then becomes the concrete mix called callus which is soft bone.

This will then change to the hardened concrete which is hard bone.

The plaster cast is not applied directly to the bone unlike the RC in a building. hence the patient will have pain during the initial setting phase with movements.

This is why the fracture treatment has progressed for pain relief and early return to function by surgery.

RC of our body: bone, fracture, weakness and how to prevent


When the building breaks with the RC weakened with time or force, one has to repair it or reinforce it, or sometimes replace it

Bones also do break as the its RC can become weak with time or otherwise called age of the person. This is called Osteoporosis.

In some instances the building was not done with good standard RC, then the building collapses prematurely.

Bones also break when its RC is defective. This happens when the defect is in the collagen namely the steel. These happen in children when they break the bone repetitively and with unusually small falls or without any injury.

Bone break when the hydroxyapatite is not not good. as you know the hydroxyapatite is made of calcium and phosphate. this is commonly seen in Vitamin D deficiency.


How then to look after this side of the bone which breaks.

1.Please read my earlier blog on Sunshine vitamin which is Vitamin D. get at least half an hour of exposure of sunlight to your body.

Remember our old tradition of applying oil to our skin and our parents made us to stand in the sunlight on sundays, may be they knew the science behind this.

Eating vegetables is good for the calcium and milk and milk products for the vitamin and calcium

2. as your parent gets older, understand that they are prone for fractures. get them to walk at least half an hour to one hour of the day.

3. if your child breaks bones often , consult a paediatrician, but remember this is very rare. Often the fact is that your child is very active and falls often which is not a bad thing.

4. Vitamin C is important for the collagen, that is steel. Fresh fruits and juices give plenty of this vitamin and keep the bones healthy

5. Both young and old, exposure to natural sunlight is important for Vitamin D. Unfortunately children do not play outside as we used to, and there is no decent place for the elderly to walk either.

As a society we need to dwell on this further
